We’re so pleased that you have chosen us to provide your medical care. We also would like to encourage all new members to make an appointment as soon as possible with the United FamilyCare physician you have chosen. It is very important to develop a doctor-patient relationship and to keep current on preventive health care.
To make an appointment, simply call the main medical group number and ask for the appointment desk. For Fontana that number is 909-822-1164; for our Rialto site it is 909-874-2371; and for our San Bernardino office call 909-887-7951.
United FamilyCare – A Physician Owned Practice
The doctors at United FamilyCare would like you to know how health care decisions are made at UFC.
UM decision making is based solely on appropriateness of care, service, and existence of coverage.
United FamilyCare does not reward practitioners or other individuals for issuing denials of coverage or service care. Financial incentives for UM decision makers do not encourage decisions that result in under or over utilization.
The primary clinical criteria used by United FamilyCare is the Apollo Medical Review Criteria, a nationally recognized, evidence based set of guidelines for in-patient, ambulatory, home health, case management, and recovery facilities care. Health plan guidelines and criteria are used as well.
Copies of specific criteria are available upon request. Policies and procedures used by UFC to authorize, modify, delay or deny services are also available to practitioners, members, member designees and the public, upon request.
